Bhagavadgītābhāṣyam

In plain words

So it follows: the last thought you hold is what determines the body you take up next —

So keep me in mind at every moment, in the way scripture prescribes. And fight — fight this battle, do the work that is yours to do. Whoever has set his mind and his understanding down in me, in Vāsudeva — that is you: with mind and understanding laid in me, you will reach me and nothing else, and you will reach me exactly as you have held me in memory. You will arrive. Without doubt — there is no doubt left here at all.

Close to the Sanskrit

Since it is thus, the final thought is the cause of attaining another body —

Therefore, at all times remember me, according to scripture. And fight — and do the battle, your own duty. He whose mind and intellect are placed in me, in Vāsudeva — that one, you, being one whose mind and intellect are placed in me, will reach me alone, just as remembered; you will arrive. Without doubt: no doubt exists in this matter.
