In plain words

Past action decides which guṇa rises in you. The guṇa that rises decides which way the mind moves. The organs — those that know and those that act, both alike — follow the mind wherever it goes, and it is through them that merit and demerit get made, here, in this life.

Close to the Sanskrit

In accordance with karma, the rising of a guṇa comes to be; in accordance with the guṇa, the mind's activity. By the organs of both kinds, which follow after the mind, merit and demerit are brought about here.
