In plain words
How so? —
Fix your mind on Me, on Vāsudeva — become someone whose whole mind rests there. Become My devotee. Let your worship be entirely Mine. And bow to Me, and only Me. With your mind disciplined, gathered into one point, you will come, in the end, to Me, the Lord — and to nothing less than your own truest Self, because I am the Self within every being, the highest goal, the ultimate refuge anyone could reach. That is what it means to be intent on Me above all else — you'll find, when you finally arrive, that you have arrived at your own Self.

How? —
You, whose mind is on Me, Vāsudeva — become one whose mind is [so fixed], [that is,] manmanāḥ. Likewise, become My devotee; become madyājī, [that is,] one whose habitual practice is sacrificing to Me. And bow down to Me alone. Having disciplined, [that is,] concentrated, [your] mind, you shall reach, [that is,] come to, Me alone, the Lord. Thus, [the object of eṣyasi is also] the Self: for I am indeed the Self of all beings, and the supreme goal, the supreme refuge; that Me, being of this very nature — [as for] 'you shall reach [Me],' the connection [of this verb] is with the earlier [word, 'mām']; being intent on Me [as the supreme goal] — such is the meaning.
