In plain words
I wish to see You again crowned, holding mace and discus. Assume that very four-armed form, O thousand-armed universal Form.
Word for word
Crowned, with mace, discus in hand, I wish to see You just as before; with that very four-armed form, O thousand-armed one, be [again], O You whose form is the universe.

In plain words
'Crowned' just means wearing a crown; 'mace-bearing' means carrying a mace; and holding the discus in hand — that is exactly how, exactly as before, I want to see you. Please. Since this is what I am asking, Kṛṣṇa says: then let me become, by that very form — the form of Vasudeva's son, with four arms — not this present form of mine that fills the universe, O you of a thousand arms. Let the universal form fold back in, and let me become, once again, that same familiar form.
Close to the Sanskrit
'Kirīṭinam' [means] wearing a crown; likewise 'gadinam' [means] possessing a mace, [and] holding the discus in hand — thus, in that very way, as before, do I wish, do I request to see you; that is the meaning. Since this is so, therefore, by that very form — the form as son of Vasudeva, four-armed — O thousand-armed one, not by the present, [now-manifested] universal form — become, O universal-bodied one; having withdrawn the universal form, become by that very form; that is the meaning.
